Long prepuce refers to the fact that when a man becomes an adult, the penis skin covers the glans, making the glans not completely exposed. However, the prepuce of immature children is generally too long, which is a natural phenomenon in development and does not belong to the category of disease. During embryonic development, the foreskin will be closely connected with the glans with the growth of the penis, and will cover the glans, which has the function of protecting the genitals. Therefore, all newborn boys will have a long foreskin, but it can protect the delicate glans, especially in the period when babies are prone to diaper rash. If you force the foreskin to pull behind the glans, it will often cause injury or even scar, and the child will feel pain. This kind of physiological phimosis will gradually disappear before the boy is 3 years old. The foreskin can retract, and can also separate from the glans to form a soft coat. At this time, it is easy to pull the foreskin behind the glans. Once the foreskin cannot be retracted after the age of 3, it is "phimosis".
After puberty, the penis develops rapidly and its volume increases, and the foreskin shrinks upward and exposes the penis head. Some people cannot turn up and expose the penis head to form a phimosis because the outer opening of the foreskin is too small. Although the outer opening of the foreskin is not small, it can also be turned up to reveal the penis head, but the foreskin is too long to cover the entire penis head and the outer opening of the urethra, which is called the foreskin is the most common congenital malformation of the male genital canal.
Excessive prepuce affects urination and easily induces infection. When the foreskin is too long, the urine flow is blocked, which makes the urine flow take on a split shape, and the clothes are wet and soft. The foreskin often secretes a smelly substance called foreskin scale. Because it can not be cleared in time, it creates a good condition for the growth and reproduction of bacteria, causing penile foreskin inflammation. Erosion of penis head, redness, swelling, pain of prepuce or urethral orifice, and purulent secretion appear. Prolonged and repeated inflammation can cause urethral stricture and further increase difficulty in urination. Because the urine produced by the kidney can not be discharged smoothly, the expansion of the renal pelvis and ureter is formed for a long time, which is called hydronephrosis. Bacteria can also tilt upward into the bladder, ureter or kidney, causing urinary tract infection. Hydrosis and infection often impair the function of the kidney. The foreskin scale and sediment in the urine accumulated together to form a bean-sized foreskin stone, which is easily mistaken for a tumor and reflects the patient's nervousness.
Don't ignore the harm of excessive prepuce in adults
1、 Obstruction of penis development: In adolescence, because the penis head is tightly wrapped by the foreskin and cannot be stimulated by the outside world, the development of the penis head is greatly constrained, resulting in a significantly smaller circumference of the crown of the penis head after the maturity of the sexual organs, which affects sexual pleasure.
2、 Cause penis inflammation: there are abundant sebaceous glands in the foreskin, which can secrete a large amount of sebum. Too long foreskin will make the secretion of sebaceous glands in the foreskin unable to be discharged, and the sebum and sediment in the urine will synthesize the cheese-like and smelly "foreskin scale". The foreskin scale is suitable for bacterial growth, so it can cause inflammation of the penis and foreskin.
3、 Damage to renal function: inflammation of the penis can cause urethral orifice or anterior urethral stricture, resulting in difficulty in urination. If urination is difficult for a long time, the function of the kidney will be damaged.
4、 Cause premature ejaculation: due to the long prepuce, the glans can not contact with the outside world well, so that the local nerve sensitivity is quite high. Once you have sex, you will ejaculate after rubbing twice. If you do this for a long time, you will form habitual premature ejaculation.
What is the reason for the long prepuce?
There are four main reasons for male foreskin to be too long:
(1) The male prepuce is too long, because the injury of the prepuce of the penis leads to the hematoma of the penis, organic adhesion, or burn scar, etc., which causes the prepuce cannot turn up and the penis head cannot be exposed.
(2) When the male penis is relatively lack of energy for upward growth, it will also cause the foreskin to be too long.
(3) The reason for the long prepuce may also be the acquired inflammatory adhesion of the penis prepuce.
(4) When the male foreskin lacks the flexibility, the narrow foreskin mouth will also cause the foreskin to be too long.
What should I do if the foreskin is too long?
Too long foreskin is a normal phenomenon in the development of children. Don't worry too much. If it affects urination and does not recur, it must be treated, and the earlier the treatment is, the better. Too long prepuce in children is very harmful. Parents and friends should be cautious, find it in time, and treat it as soon as possible. Generally, there are three cases requiring surgical treatment:
1. Acquired phimosis with fibrous narrow ring at prepuce opening;
2. Repeated onset of prepuceitis of penis;
3. After the age of five or six, the foreskin mouth is severely narrow, with difficulty in urination, the foreskin can not shrink and expose the penis, and the conservative treatment effect is not satisfactory. One of the first two conditions must be operated on. Generally speaking, prepuce surgery should be considered after puberty when patients do not need general anesthesia. However, if the little boy has recurrent inflammation or even urinary tract infection due to the long prepuce, he should be treated with surgery as soon as possible to remove the prepuce. Whether a newborn boy should undergo circumcision or not should be considered carefully because of many disputes and should not be decided hastily. If the baby has hypospadias, the foreskin should not be cut off, because the repair of hypospadias requires the foreskin.
