Glans disease is not a sexually transmitted disease
In outpatient work, there are often young men who seek medical attention due to a row or several small bead shaped papules with large needle tips growing on their glans. They are worried, some of them worry about developing tumors, and some are afraid of getting sexually transmitted diseases. In sexual medicine consultation letters, there are not a few people who ask about the lesion, and they describe it very specifically: the size of a small grain of rice in a rash, which is similar to the color of the surrounding skin, and is not painful or itchy. Although it has no impact on urination or sexual life, psychological stress is very high. In fact, this disease is neither a tumor nor a sexually transmitted disease.
Medical experts have confirmed through pathological sections that its structure is normal connective tissue hyperplasia.
In medicine, it is named pearl shaped penile papulosis. The etiology of pearl shaped penile papulosis is not yet clear, and the judgment mainly depends on the location and morphology of the rash distribution.
There is no specific treatment for this disease, nor does it require treatment. The most important thing for people with this disease is to recognize the disease and free themselves from fear and anxiety. This disease has no impact on health and the physiological function of the penis.
Leukoplakia
"White spots of varying shapes can appear on the glans, some completely white, some relatively white, painless, and not itchy. The surface is smooth without other rashes, and the surrounding pigment is often darker than normal skin tone.". There is no specific treatment for this disease, but it can often heal without treatment. "Due to the fact that white spots on the glans are associated with chronic irritation of foreskin scale, and there is a possibility of malignant transformation of white spots, those with phimosis or long foreskin should undergo circumcision as soon as possible.".
Proliferative erythema
"The appearance of a single or multiple erythema on the glans can be circular, annular, or irregular, with clear boundaries, slightly high and hard, and a bright surface.". Sometimes there may be erosion, crusting, or papillary hyperplasia. The disease may remain unchanged for many years or may become cancerous. The etiology of proliferative erythema is unknown, but it mainly occurs in patients with excessive foreskin, which is believed to be related to excessive foreskin length and stimulation of foreskin scale. The author has performed circumcision surgery on several patients with this disease, and several of them had their erythema disappear naturally. Therefore, circumcision should be routinely performed in patients with this disease. There are currently no affirmative opinions on the treatment of proliferative erythema. Some scholars advocate excision of the foreskin and close observation; Some doctors advocate freezing or X-ray irradiation of the lesion.
Patients who have erythema on their glans should not avoid medical attention. Instead, they should seek medical attention and undergo circumcision at the urological department of the hospital in the morning.
Cuticular horn
It is manifested as a horn-shaped protrusion with the thickness of a match stick or chopsticks growing on the glans, which is brown or brownish gray and has a hard texture. It is believed to be a vegetative organism formed by the proliferation of epidermal keratinocytes on the basis of glans inflammation. Cutaneous horn is considered a precancerous lesion. The characteristics of the cutaneous horn are prominent and diagnosis is not difficult. The treatment was local excision. It should be pointed out that patients with glans cutis should seek medical attention as soon as possible. Do not be embarrassed or attempt to remove it yourself, in order to avoid aggravating the condition or delaying diagnosis and treatment, leading to serious consequences.
Angioedema
Like urticaria, it is an acute allergic reaction caused by the glans to food, drugs, or insect bites. This disease can occur only in the glans, or it can occur simultaneously with urticaria. Children are prone to glans edema, characterized by a high degree of edema and shiny foreskin at the glans, similar to large blisters. But it does not affect urination. This disease often occurs at night, with no significant itching sensation, and no general discomfort. It can resolve itself after a few days. "The diagnosis and treatment of glans prepuce vascular edema is simple and easy. Local areas should be kept clean and dry, and if necessary, the lesion can be lightly wiped with 1:1000 bromogeramine.". Patients with systemic urticaria can be treated with oral chlorpheniramine and diphenhydramine, but they should be treated under the guidance of a doctor.
Fixed drug eruption
Often, after taking sulfa, barbiturates, and antipyretic and analgesic drugs orally, they suddenly develop symptoms within a day or even a few minutes of taking the drugs. "The appearance of a round or oval plaque at the glans is variable in size, with a purplish red swelling in the center and a red surrounding color. There may be itching or burning sensation locally.". In severe cases, the central area rapidly develops into a blister, which forms a wet and decaying surface after ulceration. It takes more than ten days to heal, and there is often pigmentation, which can gradually subside after several months to a year. In the future, every time a patient takes the sensitizing drug, the rash on the affected area will recur, and the area will become larger and larger. The diagnosis of this disease is not difficult, and it is easy to diagnose if there is a history of medication and the above symptoms, especially if there is a history of recurrence. "It is not difficult to distinguish between fixed drug eruption of the glans and vascular edema. There is a history of medication, and the more severe cases are fixed drug eruptions.".
Prevention of fixed drug eruption is more important than treatment. Patients should keep in mind the medications they are allergic to, actively explain them when seeing a doctor, and never reuse those medications. Once the onset of the disease, it is necessary to immediately stop taking sensitizing drugs. In mild cases, antibiotic ointment can be used externally and diphenhydramine can be taken internally for treatment. For severe cases, seek medical attention as soon as possible and seek medical attention.
