From the mechanical point of view, retrograde ejaculation may be caused by two conditions, one is the cause of weakness of bladder neck paralysis, and the other is the cause of abnormal resistance of urethral membrane.
The specific factors of retrograde ejaculation are as follows:
(1) Congenital factors: mainly including congenital wide bladder neck, congenital urethral valve or diverticulum, and congenital spina bifida. These congenital diseases lead to incomplete closure of bladder neck and increased resistance of urethral membrane, resulting in retrograde ejaculation.
(2) Iatrogenic factors: mainly including various bladder neck and prostate surgery, thoracic and lumbar sympathectomy, retroperitoneal extensive lymphadenectomy and other pelvic surgery, resulting in nerve resection or injury, incomplete closure of bladder neck, and retrograde ejaculation.
(3) Mechanical factors: Traumatic and inflammatory urethral strictures are caused by increased urethral resistance, resulting in obstruction of semen during ejaculation. Traumatic pelvic fracture can often cause posterior urethral injury and stricture. At the same time, the fracture fragment can damage the structure of bladder neck, resulting in poor closure of bladder skin and neck and retrograde ejaculation. In addition, long-term difficulty urinating can also cause a decrease in bladder neck tension, resulting in weak closure.
(4) Disease factors: The most common cause of peripheral neuropathy resulting from retrograde ejaculation is diabetes, which is usually accompanied by progressive decrease of semen volume during slow ejaculation. Spinal cord injury can make patients lose the ability of ejaculation or retrograde ejaculation, and the incidence rate is high. The most common site of injury is in the 6th thoracic vertebra, 3rd lumbar vertebra, or cauda equina nerve.
(5) Drug factors: Taking a-neneneba adrenergic receptor blockers, such as reserpine, etodol, and ornithamine, can cause muscle contractility weakness and retrograde ejaculation.
(6) Special factors: Some reasons are unknown.
