Gonorrhea is a sexually transmitted disease caused by Neisseria gonorrhoeae (Neisseria gonorrhoeae for short) and characterized by pyogenic infection of the genitourinary system. The incidence rate of STDs ranks second in China. Neisseria gonorrhoeae is a Gram negative diplococcus that is difficult to survive without the human body. Generally, disinfectants are easy to kill. Gonorrhea often occurs in sexually active young men and women.
In recent years, there has been a significant increase in gonorrhea worldwide. Since 1975, gonorrhea has recurred in China, and the number of patients has been increasing year by year, making it the main type of sexually transmitted diseases. In recent years, with the significant increase in syphilis cases, gonorrhea cases have been decreasing year by year. However, gonorrhea is still a common sexually transmitted disease in China, and it is also a key prevention and treatment category B infectious disease stipulated in the Infectious Disease Prevention Law of the People's Republic of China.
What can gonorrhea be transmitted through
1. Sexual contact infection
99% -100% of adults, especially male gonorrhea, are sexually transmitted infections. The transmission speed is fast, the infection rate is high, and the disease can occur 3-5 days after infection. Young adults are the center of infection.
At present, dark prostitutes are the main source of infection in China. Survey data shows that among gonorrhea patients, the infection rate of male and female sexual intercourse is 22% -35%, and the infection rate of both males and females is more likely to be infected by males.
2. Non sexual contact infection
This situation is rare, mainly due to contact with the patient's secretions containing gonococcal bacteria and contaminated tools, such as towels with secretions, foot pads, clothes, toilet loops, etc. Especially in women (including young girls), the urethra and reproductive tract are short and prone to infection.
Introduction to Conventional Therapy for Gonorrhea
1. General therapy
(1) Sexual isolation: Prohibition of sexual activity.
(2) Rest: STD patients with high fever and severe comorbidities should rest appropriately and, if necessary, rest in bed.
(3) Maintain a balance between water, electrolytes, and sugar water compounds, and supplement a high sugar and protein diet.
2. Systemic therapy
(1) Evaluation and indications of shower medication: There are many shower medications, but they should be selected based on the principles of efficiency, safety, and appropriate price.
① Penicillins: Destroy bacterial wall synthesis and exert bactericidal effects. This type of drug is suitable for the treatment of gonorrhea caused by non penicillinase resistant Neisseria gonorrhoeae (PPNG), which is a common cause of this disease; Standard Therapy;. However, not conducting drug sensitivity tests before treatment and using them as routine therapy is not appropriate. When the prevalence of PPNG strains is greater than 5%, penicillin based treatment should not be used, but other formulations should be chosen. The purpose of taking probenecid is to slow down the excretion of penicillin from the kidneys, reduce its binding to plasma proteins, increase blood levels, extend half-life, and fully exert its antibacterial effect.
