Sexual Health
What is the reason why the elderly have organic impotence? What are the indirect causes of organic impotence
With the growth of age, impotence in the elderly increases with the growth of age. According to relevant data, the incidence rate of the elderly over 70 years old is about 30%. The organic factors of impotence in the elderly increased significantly. In addition to a significant reduction in the number of times of sexual life, the quality of sexual life (sexual interest) of the elderly also decreased significantly.
The causes of sexual dysfunction are physiological and psychological. Impotence includes functional impotence and organic impotence. Organic impotence can be divided into direct causes and indirect causes.
Direct causes of organic impotence:
① The level of sex hormone secretion in sexual organs decreases: the level of human sex hormone directly affects sexual desire and sexual function, and the level of plasma testosterone decreases with age.
② Vascular obstruction: because the elderly are prone to atherosclerosis, such as atherosclerosis involving the dorsal penile artery and the deep penile artery, which will lead to ischemia and hypoxia, intimal hyperplasia, fibrosis or calcification, lumen stenosis will aggravate vascular embolism, and penile sponge will shrink.
③ Obstruction of venous drainage: Because the cavernous body is excessively drained by veins, such as congenital or iatrogenic fistula between the cavernous body of the penis and the glans, the white membrane venous malformation cannot maintain the penis erection. ④ Arteriovenous fistula: due to the short circuit of pudendal blood vessels and arteries, the cavernous sinus of penis cannot be filled, resulting in organic impotence.
Indirect causes of organic impotence include:
① Diabetes: The incidence rate of impotence in diabetes patients is 2-5 times higher than that in normal people. According to statistics, nearly 60% of diabetes patients have impotence, but their clinical manifestations are different. The exact reason is not clear and is generally believed to be related to the following reasons. After the occurrence of diabetes, the autonomic nerve fibers may swell and burst, and the air foam and the diameter of the axon may change. Neuropathology also found that the blood vessels supplying autonomic nerves in diabetes patients had pathological changes, which affected the nutrition supply of nerves. Therefore, the nerve reflex function is reduced, leading to impotence.
② Cardiovascular and cerebrovascular diseases: In recent years, the incidence rate of cardiovascular and cerebrovascular diseases has increased significantly. The main risk factors leading to cardiovascular and cerebrovascular diseases are hyperlipidemia, diabetes and hypertension. These diseases are closely related to people's poor lifestyle, such as excessive fat intake, dislike of exercise, heavy smoking, alcoholism, long-term stress, etc. These factors are considered to be the greatest risk factors for coronary heart disease.
For patients with coronary heart disease, one is psychological factor. They think that life and sexual life are naturally impossible and impotent; Second, these risk factors will damage the systemic blood vessels, vascular endothelial cell proliferation, lipid deposition, plaque rupture, platelet aggregation, atherosclerosis and lumen stenosis, and the vaginal vessels will lead to the insufficiency of blood supply of the vaginal artery and impotence.
The elderly should use the combination of traditional Chinese and western medicine to treat organic impotence. Hormone therapy should be used for the reduction of sex hormone secretion. Those who are young, have strong requirements for sexual life, and have poor conservative treatment effects can be treated with penis prosthesis implantation, vascular surgery, and other surgical treatments.
