Functional erectile dysfunction is caused by physiological changes in the nervous system, which only manifests as the inability of the penis to erect during sexual activity; Organic erectile dysfunction is caused by insufficient arterial blood supply, venous leakage, neurological and endocrine factors.
In the past, functional erectile dysfunction accounted for 90% and organic erectile dysfunction accounted for 10%. With the progress of modern medical examination, functional impotence accounts for about 50% to 70%, and organic impotence accounts for about 30% to 50%.
Due to the different treatment methods and effects of the two types of erectile dysfunction, it is necessary to distinguish between functional and organic erectile dysfunction. From an emergency perspective, functional erectile dysfunction usually occurs suddenly and progresses rapidly, while organic erectile dysfunction usually gradually develops and progresses slowly. From the perspective of the causes of diseases, functional erectile dysfunction is closely related to psychological factors such as fear, depression, anxiety, mental trauma, guilt, disharmony in marital relationships, lack of sexual knowledge, and excessive masturbation. Organic erectile dysfunction is mainly related to anatomical abnormalities of sexual organs, trauma, drug poisoning, and certain chronic diseases. Under sexual stimulation, functional erectile dysfunction occurs when the penis is erect, but when attempting sexual intercourse, the erection disappears, while organic erectile dysfunction weakens or disappears, resulting in a lack of response to sexual stimulation. In the injection test of vasoactive substances in Corpus cavernosum penis, functional impotence generally achieved complete erection within 8-10 minutes, while organic impotence (except neurological impotence) did not achieve satisfactory erection within 8-10 minutes. Functional impotence is often treated with psychological and medication, with a good prognosis, while organic impotence is often treated with instruments and surgery, with unsatisfactory short-term and long-term outcomes.
