Chancre is one of the eight major sexually transmitted diseases that China focuses on preventing. It is mainly transmitted through sexual intercourse and can be transmitted by direct contact with secretions and pus from open genital organs. Disordered sexual relationships and unclean sexual intercourse are the high-risk groups for this disease.
Chancre is mainly transmitted directly through sexual intercourse. After infection, a small red papule usually occurs in the external genitalia after a latent period of 2-5 days. This papule quickly turns into a pustule, which ruptures after 2-3 days and becomes an ulcer. The ulcer is circular or oval in shape and has a stealthy nature, with soft but irregular edges. The surrounding skin is congested, and the basal surface is rich in granulation tissue with blood vessels, covered with purulent secretions. Patients often feel pain at the ulcer site, which intensifies contact, If it continues to expand without treatment, the lesion often lasts for one or two months and eventually heals to form scars.
Soft scabies often occur in areas such as the foreskin, penis, glans coronal sulcus, and anus. In women, they often occur in the labia, penis, labial frenulum, urethra, vagina, cervix, and anus, and occasionally in areas such as fingers, eyelids, lips, tongue, and breasts.
Clinical manifestations of scabies below the soft surface
1. Scabies under follicular soft tissue, which is the size of a needle, forms ulcers deep in the hair follicles of the external genitalia. Short soft prickly heat is a very small lesion, such as erosion caused by genital herpes, but with irregular foundations and bleeding edges.
2. Lateral acute suppurative inguinal lymphadenitis caused by soft scabies is mostly unilateral, with local redness, swelling, heat, and pain. After transverse rupture, it turns over like a fish's mouth, commonly known as a fish's mouth. In recent years, due to the early use of effective therapeutic agents, the further development of infection has been controlled, and typical soft heat rash is rare. Therefore, it is important to understand the symptoms of soft prickly heat.
3. Heterotypic soft lower scabies, also known as one-time soft lower scabies, mainly characterized by minimal damage and disappearance within 4-6 days. However, after about 2 weeks, inguinal lymph node disease may occur, which can easily be misdiagnosed as sexually transmitted lymphogranuloma or genital herpes.
4. Protruding soft scabies, concave bottom of ulcer, and bulging granulation.
5. The initial onset is inflammatory small papules on the external genitalia. After 1-2 days, pustules quickly form, and after 3-5 days, the pustules rupture and form ulcers, with a clear boundary. The ulcer is circular or elliptical in shape, with a serrated edge. There is a latent turbidity phenomenon at the lower edge, and the surrounding area is inflamed and haloed. There is a yellow lard like pus coating at the bottom of the ulcer, covering a lot of purulent secretions. Removing the pus coating can reveal bleeding.
