Circumcision is a small surgical procedure for the treatment of prepuce and phimosis. The procedure takes about 20 to 30 minutes and does not require hospitalization. The risk of surgery is low. Long foreskin cannot be turned over, cleaned, and phimosis, especially if the foreskin has already caught the glans, it should be operated on as soon as possible. If a small boy has a small prepuce opening that is difficult to turn up, the prepuce expands into a water sac during urination, or the prepuce and penis often become inflamed, even causing urinary tract infections, prompt surgery should also be performed.
Circumcision can also lead to accidents, such as too little foreskin removed to form a narrow ring, inability to expose the penis head, and too much foreskin removed to cause painful erections or upward curvature of the penis. However, these situations are rare. "Patients who are currently complicated with foreskin and penile inflammation are temporarily not suitable for surgery, and must wait for the inflammation to subside before performing surgery.".
Specific surgical methods:
Thoroughly clean the penis and perineum before surgery. Adult men need to shave off all their pubic hair, while young boys start fasting six hours before surgery. The surgical site - the penis - will undergo local anesthesia, so there will be no pain during resection. Children who cannot cooperate with the surgery need to be given basic anesthesia to keep the child quiet.
There are two main surgical methods: stratified resection and total resection, the former being more effective than the latter. The operation is divided into three steps:
1. Before injecting local anesthetics, first use gentian violet to draw a cut line along the shape of the coronary groove three millimeters below the coronary groove. After the anesthetic takes effect, cut the foreskin and outer skin along the cut line.
2. Next, it is necessary to expand the foreskin opening. The doctor will cut the foreskin opening from the dorsal side of the penis and turn the foreskin over. If there is an adhesion between the foreskin and the glans, it should also be separated. Turn it over until the penis head and coronal groove are completely exposed, and then remove the foreskin dirt. After that, circumcision of the foreskin endothelium was performed at a distance of 8-10 mm from the coronary sulcus.
3. "Cut the foreskin tissue between the inner and outer foreskins, and perform hemostasis. Finally, suture the foreskin incision with silk thread.".
Postoperative precautions
After surgery, there will be slight pain and bruises in the wound. When the pain is obvious, it can be relieved by taking painkillers. The wound can heal in about 7 to 10 days. Currently, absorbable suture is mostly used to suture wounds, so there is no need to remove the suture.
1. After surgery, two to three layers of gauze will be fixed on the lower abdomen to cover the penis like a door curtain. When urinating, it should be lifted up and careful not to get wet with the gauze. Note that the wound should be kept clean and dry after surgery.
2. If there are peeling and adhesive wounds on the penis head, some oil can be applied to avoid adhesion with gauze.
3. To prevent postoperative bleeding, a small gauze strip can also be used to compress and wrap the incision.
4. If there is a condition of penile erection, please protect the wound with one hand and firmly pinch the painful glans with the other hand to allow the penis to subside naturally to prevent the wound from cracking.
5. If the wound is swollen, painful, or bleeding, local cold compress and pressure can be used to stop the bleeding; If bleeding is severe, return to hospital immediately.
6. Prepare several pairs of underwear for washing to maintain local cleanliness and dryness.
7. The surgery took a month to resume sexual activity.
8. Prior to wound healing, regular follow-up visits should be conducted.
Clinical experts point out that after circumcision surgery, careful care is required to achieve better recovery. Special nursing methods should also be adopted for the care of special locations. After foreskin surgery, the diet should be light and do not eat stimulating foods. As long as there is no infection after foreskin surgery, it can generally heal within seven to ten days. However, it takes about a month for the wound to fully heal and withstand external forces, such as sexual life, but the general exercise of the body will not be harmful. After treatment, it is better to use loose and breathable underwear, while maintaining the penis head upward to reduce edema. If necessary, regular follow-up visits can be made.
