1. The body position is recumbent.
2. Use soapy water and salt water for local cleaning and disinfection, and use 1:1000 bromogeramine solution for disinfection; The phimosis patient injected bromogeramide solution into the skin for disinfection with a syringe and a vein incision needle.
3. For separation of adhesions with narrow prepuce mouth and penis head adhesion, first use hemostatic forceps to expand the prepuce mouth, and then use two hemostatic forceps to clamp up the middle part of the dorsal edge (the distance between the two forceps is 0.2cm). Use the slotted probe to separate the adhesion until the penis head is completely separated from the foreskin. Then clean the foreskin sac and penis with sterile saline.
4. Design the incision and clamp the prepuce with a pair of hemostatic forceps to lift the prepuce. Use the tip of the knife to make all marks at the place 0.5 cm away from the distal edge of the coronary groove on the outer plate of the prepuce, and prepare it as a circumcision incision to prevent excessive excision.
5. For dorsal incision, use scissors to cut the inner and outer plates of the prepuce along the probe groove and align them. The inner plate of the prepuce should also be cut to about 0.5cm from the edge of the coronal edge groove.
6. Remove the prepuce, align the inner and outer plates of the prepuce, pull out the hemostatic forceps clipped at the back of the prepuce and at the laceration, and recheck whether the cut marks of the outer plate of the prepuce are appropriate as the circumcision incision. If appropriate, cut the right skin flap along the incision about 0.5cm from the coronary groove with a curved shear, and then cut the left side. The inner and outer plates of the prepuce laceration can be left uncut or more reserved.
7. Hemostasis will shrink the skin of the penis upward, and stop bleeding after exposing the bleeding point. Special attention should be paid to ligating the superficial dorsal vein of the penis in the middle of the dorsal side of the penis.
8. Suture with fine thread at the back, abdomen, left and right of the circular incision. The ligation should not be too tight to prevent the skin from being damaged when tissue edema occurs. The seam should not be cut short and should be reserved for fixed dressing. Then stitch 1~2 stitches between every two stitches, and the stitches should go out close to the cutting edge.
9. Wrapping: wrap a piece of Vaseline gauze (the raw edge is folded inside) around the incision of the foreskin, fix it with a long suture, and then wrap it with several layers of gauze.
