The skin of the penis is folded into a double layer at the glans of the penis, which is called foreskin. Before the age of 7, the foreskin is longer and can completely cover the penis and the external orifice of the urethra. Generally speaking, you can expose the glans by pushing back the foreskin of a child with your hand. The shape of the glans is a large honeycomb shape with a sharp front and a large back. The foreskin in childhood has a protective effect on the glans. With the development of puberty, the penis volume increases, the foreskin shrinks back, and the glans can be exposed naturally after adulthood. However, about 30% of men have a long foreskin of the glans, which can only be exposed when they turn the foreskin up by hand when they grow up, and 8% of men have a foreskin mouth that is too small and tight, and the foreskin cannot turn up at all, which is called phimosis.
Children's phimosis or foreskin is too long. When children start to metabolize and excrete normally after birth, phimosis or foreskin has a certain impact on urination, especially when phimosis occurs, the foreskin will bulge like a balloon when children urinate. After urination, there is often a little residual urine in the foreskin cavity, and the accumulated sebum secretion and epithelial desquamation will gradually form massive foreskin scale, This is a good place for bacteria to reproduce and grow. Long-term chronic stimulation can cause infection in the prepuce cavity, forming prepuce balanitis, and repeated attacks can cause adhesion in the prepuce cavity. Sometimes you can feel the hard lumps under the foreskin. Due to the presence of foreskin scale and the adhesion between foreskin and glans, it often causes the whirlpool of urination and impeded urination. There may also be redness and swelling and many secretions in the local area.
Pediatric phimosis should be treated as soon as possible. There are two common methods. First, when there is no adhesion, the operation method after local anesthesia can be used to expand the prepuce mouth by artificial expansion or inflatable expansion; The second is to take circumcision when there is adhesion, especially when the adhesion is serious. In case of adhesion, the shape of the glans will be uneven and the color of the pigment will vary after excision, which will affect the appearance.
Therefore, for children with phimosis, in order not to affect the normal development of the glans, circumcision can be performed as soon as possible. For patients with long foreskin, it is not necessary to rush for surgical treatment, as long as the foreskin is often turned up and cleaned, but attention should be paid to timely reposition the foreskin after turning up and cleaning, so as to avoid the formation of foreskin incarceration. The foreskin operation is not big, but careless operation will also have adverse effects on future sexual life, so it should be treated with caution.
